Important Updates

Check your 457 Plan Beneficiaries
Remember to check your beneficiaries for the NES 457 deferred compensation plan. Prudential is now the record keeper of all 457 beneficiary information. Your beneficiary information is available on the Prudential website, www.prudential.com/online/retirement, under Account Detail, Personal Information. Please log in and review your information and make any changes, if needed. You may also contact Prudential at 877-778-2100.  

Change for Vision Plan Claims
HealthSCOPE Benefits has updated its policies regarding the Vision Plan. All vision claims must include the doctor’s prescription and type of lenses or contacts dispensed. If the Explanation of Benefits (EOB) indicates that a claim was denied for this reason, submit a copy of the prescription for the lenses and the EOB to HealthSCOPE at the Lubbock, TX address on your insurance card. It will be considered a new claim, and a decision will be made within 30 days. For assistance, contact HealthSCOPE at 877-385-8816.

Walmart Vision Center Billing Changes
Walmart and Sam's Club have changed their insurance billing system and no longer send claims to HealthSCOPE for vision services under the NES Vision Plan. Walmart will be collecting payment from you at the time services are rendered, including eyewear purchases. Ask for an itemized receipt and submit it along with a completed vision claim form below (Part 1 only) to the HealthSCOPE address listed on the form. You will be reimbursed by HealthSCOPE at 85 percent of the eligible charges, up to $450 per calendar year per covered person. If you have any question, call Compensation and Benefits at 747-3942.

HealthSCOPE Benefits Online Account Changes
Due to HIPAA privacy compliance, changes have been made to the HealthSCOPE website that impact retirees. Spouses and all adult dependents of an insured are now required to have their own online account. The Primary Insured no longer has access to claims and coverage information for their spouse or adult dependents.
